hereby submit my application for the participation in Dubai Budokan Cup – International Karate Championship 2023. I admit that there are risks of injury involved in participating in the tournament. It’s my responsibility to take care of myself.  I hereby waive release any and all claims, by cause of act ion, damages, cost, expenses, either known or arising in the future, I will not stand against any tournament organizers or officials afterwards.

By signing this document, I acknowledge that I have read the rules and regulations and agree to abide by them and assume full responsibility, for any and all my action during this tournament.
